• Functional Medicine New Patient- $450 (Includes Initial Nutrition Consult)
• Functional Medicine- Existing Chiro Patient- $350 (Includes Initial Nutrition- Pt. needs FM paperwork)
• Functional Medicine Comprehensive Lab Review- $90 (Follow-up from Initial Visit)
  - Includes Specialty Lab Review and Treatment Plan
• Functional Medicine Follow Ups (Package of 4)- $320 ($80/visit) Typically at 2-3 months post initial visit, then every 3-4 months.
• Functional Medicine Follow Up- $90 (Typically at 2-3 months post initial visit, then every 3-4 months)
• Basic Lab Review- $70 (Basic bloodwork review prior to Comprehensive Labs)

Nutrition Fee Schedule

• Nutrition New Patient- $275
• Nutrition New Patient with Significant Other- $350 ($275 + $75)
• Nutrition Comprehensive Follow-up (45 minutes)- $150 (not seen in over 1 year)
• Nutrition Basic Follow-up (30 minutes)- $65
• 6 Visit Nutrition Package- $360 ($60/visit)- Does not include Initial Visit (Recommended for FM pts)
• Nutrition Lab Interpretation- $70 (additional fee for Dr. K to review lab results ordered by Cassie)
  - $90 for Dr. K to interpret and review Functional Medicine lab results with patient

For Patients Needing Added Accountability/Patients Outside of Central Oregon

• Nutrition & Lifestyle Coaching (optional)- $99/month (3 month minimum)
  • Includes 1 physical check in per month as well as unlimited email support
• Nutrition & Lifestyle Coaching (optional)- $75/month with 12 Month Commitment
  • Includes 1 physical check-in per month via office or phone
  • Unlimited email communications
  • Nutrition & Lifestyle Coaching options require Initial Nutrition Consultation
